Proof of Identity (photo ID such as a driver's license, passport, etc.) of the person giving the samples is required at the time of sample collection.
A photo of the person giving the sample is taken.
The health care professional provides a sworn statement affirming that he or she collected samples from the identified person.
The health care professional forwards the samples by express courier in sealed protective pouches to the AGI laboratory.
